Your annual bloodwork was never built to see this.

Why this exists.

Routine bloodwork leaves biological risk invisible. The model it belongs to is reactive — organised around volume rather than around high-touch care.

Most people will die from one of four diseases — cardiovascular disease, cancer, neurodegeneration, or metabolic dysfunction. The challenge is not that these conditions exist, but that their earliest biological signals are often detectable and modifiable before symptoms arise. Understanding these mechanisms early, at both the molecular and cellular level, is the foundation of true longevity.

What the membership is

Longevity medicine run as a year-round system.

A single set of results is a document. It tells you where you stood on one morning, and it ages from the day it is printed.

The membership exists to do the other thing: hold your physiology in view across a year, watch the direction each marker is moving, and adjust while the trend is still a trend. One physician does the reading. It is the same physician every quarter, and he already knows what your last four panels said.

Concierge members can have Dr. Bajnath serve as their primary care physician, with unlimited access.

Institute for Human Optimization — stated policy

Included in the annual membership

  • Annual or baseline Biological Audit® for new patients
  • Quarterly laboratory assessments
  • Quarterly VO₂ max and performance evaluations
  • Mobile phlebotomy within thirty miles of 21076
  • Direct physician access and concierge patient coordination
  • Premier Longevity Gyms access, with physician-guided recommendations
  • Preferred pricing on weight loss, hormone, peptide and regenerative programs

Peptide and hormone therapies are prescribed when medically appropriate and legally permitted within your state.

Seven layers of one person.

The Biological Audit® reads more than 250 precision biomarkers across all seven, then interprets them as one system rather than as seven reports that never meet.

  1. Genomics

Whole genome sequencing with clinical interpretation. The sequence does not change, so it is read once and then referenced against everything after it.

  2. Epigenomics

Which of those genes are switched on now, and the biological age that pattern of expression implies.

  3. Transcriptomics

The instructions your cells are issuing at the moment the specimen is taken.

  4. Proteomics

The proteins actually built from those instructions, which is not always what the instructions predicted.

  5. Metabolomics

The output of the machinery: what your metabolism is producing while all of the above runs.

  6. Microbiomics

The bacterial population carrying out a share of the work, and what its composition is doing to the rest.

  7. Exposomics

Everything outside you that has left a mark inside: toxin burden, environment, the accumulated load of a life.

The genome is sequenced once. What it means for you is re-read every quarter.

The year, quarter by quarter

What a year actually asks of you.

Four calibrations, spaced across twelve months. This is the whole commitment, written out — read it before you apply rather than after.

Quarter one

The baseline

The Biological Audit®: a head-to-toe multi-omic evaluation, including whole genome sequencing with clinical interpretation. Roughly two months end to end, with four to six weeks of analysis from the day the lab receives your specimen.

  • Consultation
  • Specimen collection
  • Results review

Quarter two

First calibration

Laboratory assessment and a VO₂ max evaluation, read against the baseline rather than against a reference range. The first quarter where a direction of travel exists at all.

  • Labs
  • VO₂ max
  • Physician review

Quarter three

Second calibration

The same panel again. Three points make a line, and this is the quarter where a marker that is drifting becomes distinguishable from a marker that had one unusual morning.

  • Labs
  • VO₂ max
  • Plan revision

Quarter four

The year in review

Fourth assessment, and a full read of the twelve months against the baseline. The precision plan is rewritten for the year ahead on what your own biology did, not on what the average does.

  • Labs
  • VO₂ max
  • Annual strategy

Specimen collection is matched to your panel. Members within thirty miles of the Hanover office can arrange collection at home or at work.

Eligibility

Fifty patients a year, and the reason that number is real.

It is not a scheduling preference. Reading 250 biomarkers against a genome, four times a year, is physician hours — and there is one physician. Existing members hold priority for the following year, so the number of places that open is smaller than fifty.

Five conditions, and all five have to be yes.

Who each one rules out

  1. Billing

You can pay for care directly, and you treat any insurance reimbursement as a bonus rather than as the plan.

    Not if

You need insurance to bill directly. This practice is out-of-network and cash-based. Superbills are provided; reimbursement is never guaranteed.

  2. Scope

You want one physician holding the whole strategy, not four specialists holding a quarter of it each.

    Not if

You want one procedure — a peptide protocol, hormone therapy, a regenerative treatment. That is a sixty-minute suitability consult, not a membership.

  3. Horizon

You accept that a trend takes four quarters to become readable, and you are willing to wait for it.

    Not if

You want a single answer rather than a year of oversight. The Biological Audit® at $2,500 is a complete one-time evaluation and is not a prerequisite for anything.

  4. Participation

You will give four laboratory assessments and four performance evaluations across the year, you can be reached, and you will act on what the data says.

    Not if

You want to hand a problem over and collect a result. This is a year of joint work; the data is only worth what you do with it.

  5. Certainty

You want the direction your markers are moving. You do not need a promise about where they land.

    Not if

You want a guarantee of an outcome. No physician can offer one, and this page will not pretend otherwise.

Membership also requires that you are within reach of Hanover, Maryland, or willing to travel there.

This is not emergency or urgent care and does not replace it. For an emergency, dial 911.

Applying

Request a consultation.

The consultation establishes whether a year of this is the right use of your time and money. If it is not, you will be told so, and pointed at the thing that is.

$14,500
Per year · Concierge Longevity Membership
  • Out-of-network and cash-based. The practice does not bill insurance directly. Superbills are provided for submission to your plan; reimbursement is not guaranteed.
  • Many patients use HSA or FSA funds for eligible services and diagnostics. Eligibility varies by plan.
  • Regenerative therapies — PRP, exosomes, EBOO/EBO₂, ozone — are billed separately from program fees. Members receive preferred pricing.
  • All fees are non-refundable, given the personalised nature of the work.

Concierge application

Six questions · Reviewed by Dr. Bajnath

One form for both: say in question five whether you want the Biological Audit® or the membership.

Submitting an application does not create a physician–patient relationship. Places are limited to fifty per year and existing members hold priority.

Not ready for a year? The Biological Audit® is directly below, at $2,500, or $5,000 for the Elite panel.

The other door

The Biological Audit®, on its own.

It is a complete one-time evaluation and not a prerequisite for anything — the membership is an optional continuation of it, not the reason to do it.
